Win tickets to see JLS

Hi! We're working with a specialist team working across all of County Durham and Darlington to provide free and easy self-taken tests for a sexually transmitted infection called Chlamydia. There's a lot of it around, about 1 in 10 young people who have ever had sex will have Chlamydia.
Don't worry if you've got it, we can help you sort it.
If you are under 25 years old and have ever been sexually active you can have a test. Our programme is designed to test those who currently have no signs or symptoms. You may also have the test if you are under 16 years old.
We are sorry but if you are over 25 years old you will not be able to take part in this screening programme, unless your partner has been part of this scheme. If you have symptoms or would like to be tested for other sexually transmitted infections including Chlamydia contact your local Department of Genito-Urinary Medicine (GUM Clinic) or your G.P.
Request a testing kit and be entered into a draw to win tickets to see JLS at The Metro Radio Arena!
Please note: By entering this competition you are agreeing to have a testing kit posted out to the address you enter below.
If you are requesting a postal pack for someone else please ensure you have their permission. If you are doing this as a joke or hoax it could lead to an investigation. We will keep your details in a confidential file in case of any such queries.
If you are taking the test in a clinic the staff will be able to help you. You must provide your name, address and date of birth. Just because you have given your address it does not mean that we will write to you at your home, these details are required by the programme but not passed on to anyone else.
